RELEASE OF LIABILITY FORM

RELEASE OF LIABILITY, WAIVER OF CLAIMS, ASSUMPTION OF RISKS AND INDEMNITY AGREEMENT WARNING: BY SIGNING THIS AGREEMENT YOU, YOUR FAMILY AND REPRESENTATIVES WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE.

PLEASE READ CAREFULLY!

For the purposes of this Agreement, the Amazon Explorer SERVICE (the Operator) will begin from the initial moment of the itinerary in Iquitos, Peru; and will conclude as indicated therein.
On behalf of myself, my heirs, next of kin, executors, successors, administrators, assigns and whomever else may have an interest either at common law or by operation of statute, I hereby waive any and all claims I or such parties have or may have in the future against the Operator or Releasees, and I release from all liability and agree not to sue the Operator or Releasees for any and all personal injury, death, property damage, expense or loss sustained by me or my next of kin as a result of my participating in this Expedition and/or Survival Training with the Operator, due to any cause whatsoever including breach of contract or changes to the itinerary that the Operator is forced to carry out due to any force majeure or act of God, and also including the failure on the part of the Releasees to safeguard or protect me from the risks, dangers and hazards of this adventure referred to above.
I CERTIFY that I am in good physical condition and that there are no reasons or problems related to health that prevent my participation in this Expedition and/or Survival Training.
I acknowledge that the enjoyment and excitement of expeditions are derived in part from the variable unpredictable terrain, and that the inherent risks of adventure travel contribute to such enjoyment and excitement. I understand that if I suffer an accident/injuries, the guides and other team members will make all possible efforts for my health care. I also accept not to blame aforementioned individuals to be responsible for any additional injuries resulting from any first aid measures.
I fully realize that these adventures are extreme and risky; I shall therefore be responsible of any accident, injury or illness caused by any insects or any animals from the region. I fully understand all the possible risks and dangers that exist in these areas.
I accept that any infection or illness acquired in these areas shall be my sole responsibility. I shall also assume my own responsibility in case I decide to participate in any shamanist traditional ceremony and accept any consequences resulting thereof.
By signing this form, I also agree to provide my own medical and life insurance while I take part in this Expedition and/or Survival Training. I also understand that the Operator will not assume any financial responsibility for medical or evacuation expenses that may occur.
In case that due situations beyond the control of the Operator, as strikes, street block, etc., the Operator will try to make everything possible in order to do not affect the itinerary, nevertheless I shall assume my own responsibility in case the start of the expedition is delayed, and too I understand I will not receive any kind of reimbursement from the Operator for the days lost.
I shall assume my own responsibility in case the return trip is delayed due to situations beyond the control of the Operator and as a result, I miss my return flight. Likewise, if I voluntarily decide to return before concluding of my Expedition and/or Survival Training, I will not receive any kind of reimbursement from the Operator.
I accept the laws and regulations of the Peruvian Government during my stay and that I agree not to infringe these laws nor any other international laws while taking part in this Expedition and/or Survival Training.
I understand and agree that, during this Expedition and/or Survival Training, I agree to always remain with the team and project a friendly attitude towards indigenous people and members of the group. I agree at all times to comply with the suggestions and recommendations of the Operator’s guides and Tour Leader.
I CERTIFY THAT I HAVE READ THIS DOCUMENT AND THAT I COMPLETELY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A DISCLAIMER AND CONTRACT AND I SIGN IT WITH MY OWN FREE WILL.
